Reimbursements are provided when patients fall off therapy through an early discontinuation credit for participating Diabetes Care Value Program medications.
Omada for Prevention will work in parallel with Digital Diabetes Solution. Omada for Prevention targets those at risk of developing a chronic condition and Digital Diabetes Solution targets patients already living with diabetes. Both programs focus on member engagement, working to drive member adherence and lower costs.
There is no member impact when a client implements Digital Diabetes Solution/Digital Diabetes Prevention and Obesity Solution, as it is at the patient’s discretion whether or not they would like to participate.
A CGM offers real-time, dynamic information about glucose levels that can paint a more complete picture of how diabetes is affecting the patient than a single blood glucose meter reading. By measuring blood sugar levels continuously throughout the day, typically every five minutes, this wearable technology helps members more easily track their blood sugar levels over time.
CGMs are also convenient to wear because a small sensor is positioned just under the skin and secured there with adhesive tape. Blood sugar data can be sent from the sensor to either a handheld receiver device, an app on a smartphone or an insulin pump.
